Consent to the personal data processing
pursuant to Article 6(1)(a) and Article 7 of Regulation (EU) 2016/679 of the European Parliament and of the Council of the EÚ 2016/679 from the 27th April of 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (General Data Protection Regulation)
as the data subject, by my signature I grant the control unit, Trenčín Alexander Dubček University, Studentská 2, 911 50 Trenčín (hereinafter referred to as "the control unit"), consent to the processing of my personal data for the purposes of:
1. to keep records and overview of individual persons who participated in the presentation, seminar, congress, etc., to the extent specified in the application /form/ in the above-mentioned scope.
This consent to the processing of personal data is a freely given, specific, informed and unambiguous expression of my will, which I grant for a period of 5 years, or 30 days after withdrawal of the consent. After this period, your personal data will be mechanically destroyed (shredded) and will no longer be processed in any database.
Personal data will not be disclosed to recipients within the scope of the above purpose.
2. making or obtaining (photographs, videos, results of creative activity, etc.), the collecting on personal data carriers (physical and data carriers), organizing, storing, providing to third parties, making available to recipients and publishing personal data, within the information system, promotion of the operator for the purpose of promotion of the operator, publication of personal data ⃰ and information about organized events, activities of the operator with the intention of building its reputation, to the extent of the following:
- photograph (color or black and white),
- video (image and/or sound),
⃰ personal data that are processed within the IS Promotion of the operator on the basis of my separate consent will be published on the following websites and in the following premises of the operator:
- University website,
- the conference website,
- the internal premises of the University,
- social networks,
- Proceedings.
This consent to the processing of personal data is a freely given, specific, informed and unambiguous expression of my will, which I grant for a period of 10 years (promotional films, photographic materials related to the University) (if technically possible). After that period, your personal data will be mechanically destroyed (shredded) and will no longer be processed in any database.
Personal data will not be disclosed to recipients within the scope of the above purpose.
Before giving my consent, I was informed of the following:
- my personal data, the provision of which is voluntary, will be kept for the period of validity of the consent and will not be processed for any purpose other than that for which it was obtained.
- As the data subject, I have the right to request from the control unit an access to my personal data concerning me, the right to rectification or erasure or restriction of processing, the right to object to processing, as well as the right to data portability.
- If justified, I have the right as a data subject to lodge a complaint with the supervisory authority, which is the Office for Personal Data Protection of the Slovak Republic.
- I acknowledge that there is no cross-border transfer of my personal data to a third country.
- I acknowledge that there is no automated decision-making, including profiling.
I may apply my right to withdraw my consent at any time, including before the expiry of the period for which consent was given, as the data subject in the following ways:
a) by sending an email request to zodpovedna.osoba@tnuni.sk,
b) by sending a written request to the address of the control unit's registered office with the text "GDPR - withdrawal of consent" on the envelope.
c) in person, at the operator's headquarters
Withdrawal of consent shall not affect the lawfulness of processing based on consent prior to its withdrawal.
I am aware of my rights as set out in Articles 12 to 23 of the General Data Protection Regulation, which regulate or specify the obligations of the controller in exercising the rights of data subjects.
