Consent to the personal data processing
According to Article 6(1)(a) and Article 7 of Regulation (EU) 2016/679 of the European Parliament and of the Council of April 27, 2016, on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, as the data subject, I agree to the following: I am giving my consent for my personal data to be processed by the controller, Alexander Dubček University of Trenčín in Trenčín, Študentská 2, 911 50 Trenčín (hereinafter referred to as the "controller"). I understand that this consent is given for the following purposes:
1. Keeping track of the people who attended presentations, seminars, and congresses, as specified in the application form, or other documentation.
I agree to the processing of my personal data. I give this consent freely, knowingly, and unambiguously. I agree to this for a period of 5 years or 30 days after revoking my consent. After this period, personal data will be destroyed (shredded) and will no longer be processed in any database.
2. Creation or acquisition (photos, videos, creative projects, etc.), collection on personal devices (physical and digital)), organization, storage, sharing with third parties, disclosure to recipients, and publication of personal data within the operator's promotional information system for the purpose of promoting the operator, publishing personal data and information about organized events and activities of the operator with the aim of building its reputation, to the extent of:
- photo (in color or black and white),
- video recording (image and/or sound)
I give my separate consent for the operator's Promotion IS to process my personal data. This data will be published on the following websites and in the following areas of the operator:
- university website,
- conference website,
- university premises,
- social networks,
- proceedings/scientific journal.
I give my consent for my personal data to be processed. I understand that this consent is freely given, specific, informed, and unambiguous. I grant this consent for a period of 10 years (for promotional films and photographic materials related to the university) (if technically possible). After that time, the personal data will be destroyed (shredded) and won't be stored in any database anymore.
Before I gave my consent, I was informed of the following facts:
- I can choose to share my personal data, which will be stored for as long as I choose to consent and will not be used for any other purpose.
- As the person whose personal data is being collected, I have the right to: ask the controller for access to my data, get it corrected or deleted, limit how it's used, say no to its use, move it to another service.
- If you have a good reason, you can make a complaint to the Office for Personal Data Protection of the Slovak Republic.
- I understand that my personal data will not be sent to other countries.
- I understand that no automated decisions are made, including decisions about what information to collect.
As the person whose personal information is being collected, I have the right to change my mind and stop giving my consent at any time. I can do this before the time limit for giving consent has passed. There are a few ways I can do this:
a) send an email to zodpovedna.osoba@tnuni.sk,
b) send a written request to the address of the controller's registered office. On the envelope, write "GDPR - withdrawal of consent."
c) in person at the Controller's registered office.
If you withdraw your consent, that doesn't change the legality of processing your data based on your consent before you withdrew it.
I am aware of my rights as outlined in Articles 12 to 23 of the General Data Protection Regulation. These articles specify the obligations of the controller when exercising the rights of data subjects.
By signing this document, the person confirms that they have read and understood the information about how their personal data will be processed. They also say that they have been told about the extra information in the GDPR and the Act, which is important for doing their jobs. They also say that they have been told that the current documents that protect their personal data can be collected at any time from the Controller's registered office.
